On May 16, 1995, I was taken to Cushing Hospital, Leavenworth, Kansas for x-ray and MRI testing. Please forward copy of the following information as per the x-ray and MRI testing:

1. X-ray report from Cushing Hospital;
   
2. MRI report from Cushing Hospital;
   
3. MRI report from technician taking the MRI for Mobile Technology Inc.;
   
4. Number of scans taken during MRI Testing;
   
5. Number of total data megabytes within the complete MRI exam;
   
6. Type of MRI machine used;
   
7. Exact procedures used to conduct the MRI exam;
   
8. Exact settings used in conducting the MRI exam; and
   
9. Copy of all reports referencing the x-ray and MRI exam taken on May 16, 1995.
